Find All (Peek) References UI randomly doesn't work as expected

I'm seeing Find All (Peek) References fail in various ways seemingly randomly and/or in combination with other UI steps, such as using the Cancel or Preview buttons. I don't have exact repro steps yet.

From time to time, this feature of vscode breaks down for a reason I can't understand.
My workaround: When I realize this situation, applying "Rebuild intellisense index" operation (accessed from help->all subjects) makes it recover.

This issue has been inactive for a long time, and we will be unable to investigate and address it in the foreseeable future.
