inbox: the mid-loop human channel

Deferred from v1 (.inbox delivery tiers): today, answering one question from the loop means
killing the Run. Field tools converged on blocking Q&A channels (approval gates, chat
bridges). Bring back the .inbox design as the mid-loop human channel.
