verify: a phase-level cost ceiling

An agentic Verify (work-tree inspection, running the build) can cost more than a near-free
transcript check — today only the Loop-total usd cap bounds it. Give the Verify phase its
own ceiling so an expensive judge can't quietly dominate the budget.
