Cover page
Try using:

  pdf:
    header-includes: |
      \titlehead{\includegraphics[width=6in]{cover-image.png}}

Simpler solution from Vincent A-B: Use pdfjam in a Makefile to simply add the cover page at the beginning of the book.
[But need to install pdfjam for windows]

copy: ## copy pdf to Dropbox
pdfjam images/cover.pdf pdf/index.pdf --outfile pdf/index.pdf

Found one solution, using the utility PDFtk. It has a GUI, but also a commandine tool, pdftk

In the pdf/ directory, I ran

$ pdftk A=images/cover-ellipse.pdf B=index.pdf cat A1 B2-end output index-cov.pdf

This concatenates images/cover-ellipse.pdf with pages 2-end of the index.pdf file.

But, doing so changes the structure of the PDF -- no bookmarks, etc.
What is needed is something like what I did manually in Adobe Acrobat Pro:

• open index.pdf,
• then use Organize Pages to insert the cover before the first page.

Use tikz to include a cover image directly from the yaml. But I would use this in the latex/preamble.tex
