Subject: [PATCH] docs: add a prominent link to the documentation site in the
 README

Add a Documentation badge to the header badge row and a "Read the
documentation" call to action directly under it, both linking to
https://commandprompt.github.io/open_pg_tde/, so the published docs site
is reachable from the top of the README rather than only from the
Documentation section lower down.
