Subject: [PATCH 1/2] Disable Claude attribution in commits and source code

Add Attribution Policy section to CLAUDE.md and codestyle.attribution
setting to prevent AI co-author tags, session URLs, and generated-by
comments from appearing in commits or source files.

Subject: [PATCH 2/2] Fix attribution settings to use correct top-level format

Replace deprecated codestyle.attribution with the proper top-level
attribution object with empty commit and pr strings.
