Subject: [PATCH] chore: capture intent-sweep follow-up anomalies as
 iss-16..iss-23

Eight record-consistency findings from the 64-intent dependency sweep:
four majors (itd-66 canon vs blocker role, itd-50 lifecycle state,
itd-37/itd-36 phase-order violation, itd-27/itd-42 direction conflict,
launch cluster unscheduled) and three minors (itd-launch name refs,
itd-6 and itd-36 supersession framing).
