finish PWA support and offline-first install flow

PWAHandler.tsx registers a service worker and prompts install, but there is no integration with manifest, SW file, or install UX docs.

I'll take this. Will wire up manifest.ts, sw.js, and PWAHandler.tsx properly icons, start_url, display mode in the manifest, service worker registration/lifecycle, and the install prompt UX (beforeinstallprompt handling, dismiss/install states). Update README with the install flow. Done this exact setup before, know the common gotchas (SW caching stale on updates, manifest not matching icons, etc).

Hey @MaryammAli — this is already done, PR merged already (fixed the PWA install flow — PWAHandler wasn't mounted, manifest icons were broken, service worker caching gaps). Should show as closed now, let me know if anything's missing.
